چکیده

Introduction The ablate-and-pace technique is a well-established method to treat patients with persistent and paroxysmal atrial fibrillation with poor rate control on medical therapy.1 The technique represents a class IIa indication to control heart rates when pharmacologic therapy is insufficient or intolerable.2 As many of these patients are in permanent atrial fibrillation, they frequently require only single-chamber ventricular pacing after an atrioventricular (AV) node ablation. With the advent of transcatheter pacing systems, single-chamber pacemakers can now be implanted percutaneously without the need for leads or creation of a pocket or transvenous leads. Leadless pacemakers have a lower complication rate compared with that of traditional pacemakers in a historical control population.3

Commentary: An Unusual Case of Leadless Pacemaker Implantation in a Patient with Wolff–Parkinson-White Syndrome: Just Because We Can, Does That Mean That We Should?

In the case presented by Drs. Tanaka-Esposito and Cantillon, a patient deemed to be at risk of developing unpredictable atrioventricular (AV) block based on electrophysiology findings and a catheter ablation procedure is chosen for the implantation of a leadless pacemaker that will provide permanent, prophylactic ventricular pacing. Briefly, this case involved a patient with a midseptal accesso...

Mortality analysis in patients with atrial fibrillation and implantable permanent pacemaker after ablation of the atrioventricular node.

OBJECT Atrial fibrillation is associated with excess mortality. This analysis was performed to determine the magnitude of the excess mortality risk. METHODS Life table analysis of a published study of 350 patients undergoing radiofrequency ablation of the atrioventricular node and permanent pacemaker implantation for symptomatic paroxysmal or chronic atrial fibrillation. A substantial proport...

Severe cardiac conduction disturbances and pacemaker implantation in patients with hypertrophic cardiomyopathy.

The aims of this study were to determine the prevalence of severe cardiac conduction disturbances in a cohort of 451 patients with hypertrophic cardiomyopathy and to describe the characteristics of, and outcomes in, those who required a permanent pacemaker. A pacemaker was implanted in 48 patients (11%): 20 had sinus node dysfunction and 28 had an atrioventricular conduction disturbance. Primar...
